Student Health Requirements
*IMPORTANT NOTICE: If your child was in one of these grade levels last year and did not submit the required documentation, they MUST submit it before they start school this year.*
Students entering kindergarten and Grade 6, or who are entering an Illinois school from out of state or out of country at any grade level, must provide proof of physical examination on the Illinois approved form.
- The exam must be completed no earlier than a year (365 days) prior to the student’s first day of attendance.
- The form must be completed by the health care provider and include evidence of all required immunizations.
- There is also a section on the back of the form at the top that must be filled out and signed by the parent/guardian.
All students in kindergarten, Grade 2, and Grade 6 are required to submit the Proof of School Dental Examination form by May 15 of that school year. The form must be completed and signed by a licensed dentist. The dental examination must have been completed within the 18 months prior to the May 15 deadline.
All students entering kindergarten, or who are enrolling in an Illinois school for the first time (not including preschool), are required to submit the Eye Examination form by October 15. The form must be completed and signed by a licensed optometrist or medical doctor who performs eye examinations. The eye examination must have been completed within one year prior to the first day of that school year.
